. The recalled chairs' legs can break, posing injury and fall hazards to consumers.

. The recalled gates violate the mandatory standard for expansion gates and expandable enclosures because a child's torso can fit through the opening between the gate and the floor, posing a risk of serious injury or death due to entrapment.

. The cord lock on the recalled water ballons' storage bags contain a regulated phthalate, which violate the federal phthalate ban. Phthalates are toxic if ingested by young children and can cause adverse health effects.

. The recalled bed rails violate the mandatory standard for adult portable bed rails because users can become entrapped within the bed rail, or between the bed rail and the side of the mattress, posing a serious entrapment hazard and risk of death by asphyxiation. In addition, the bed rails do not bear the required hazard warning labels.

The Food and Drug Administration (FDA or Agency) is announcing the availability of the draft guidance entitled "Hernia Mesh--Package Labeling Recommendations." This draft guidance provides labeling recommendations for hernia mesh devices that are intended to help promote the safe and effective use of hernia mesh. This draft guidance is not final nor is it for implementation at this time.

This proposed rule would revise the Medicare Hospital Outpatient Prospective Payment System (OPPS) and the Medicare Ambulatory Surgical Center (ASC) payment system for calendar year 2026 based on our continuing experience with these systems.
